The National Centers for Disease Control (CDC) and the federal Occupational Safety Hazards Administration (OSHA) recommend that all health care workers who might be exposed to blood or blood products in an occupational or educational setting should receive the Hepatitis B vaccine, preferably before exposures could occur. Thus, all students are strongly encouraged to obtain the vaccination. The vaccination is a three shot regime over a period of 6 months (initial, one month, six month). Thus, one should plan well ahead to have the vaccination completed before enrollment in the program. Check with your local physician and /or health department about the availability of the vaccine. All students accepted
into a Armstrong health professions program must complete
the enclosed Hepatitis B Declaration Form. There are three
options for signature. Please sign and date only one of the

| Explanation: | The
Medical Technology Department of Armstrong Atlantic State University
requires screening for tuberculosis and follow-up for reactions
>10mm of all students in program. Requirements are based on
current recommendations of the Centers for Disease Control and
Prevention (CDC) for health care workers. Pregnancy does not exclude
students from annual TB screening.
Screening for Entering First Year Students
A single Mantoux (PPD) skin test will be administered. The Mantoux skin test must be read and recorded by a licensed healthcare provider within 48-72 hours prior to student attending school. A history of Bacillus of Calmette and Guerin (BCG) is not grounds for waiving screening. A reaction >10mm will be considered positive. Students who have documented prior history of a positive TB skin test or who test positive (? 10mm) in their initial TB screening must provide documentation ruling out active disease. If student cannot provide documentation of chest radiograph within past year, one must be obtained. If student can provide documentation of radiograph within past year, student does not need to repeat radiograph. Student must have licensed healthcare provider fill out TB Screening Questionnaire (attached) and return it to the Medical Technology program. If information obtained in the questionnaire reveals symptoms suggestive of TB, the student must undergo a medical evaluation including a chest radiograph and follow up treatment as indicated. Students with positive skin tests will be required to be screened annually using the TB questionnaire. |

| Explanation: | The University System of Georgia requires that students have in-force health insurance. The University System of Georgia uses Pearce and Pearce, Inc. for it's student health policy. Students with existing coverage must obtain an insurance waiver from Pearce and Pearce by going to their website www.studentinsurance.com and giving your current insurance information. Pearce and Pearce will email back that they have received your information. You will receive a second email within 2-4 days stating that your information has been verified and approved. This email must be forwarded to the Medical Technology Department at medtech@armstrong.edu. If your waiver is declined there will be a reason given. If you disagree with this decision, call the Customer Service Line to speak with a representative. (1-888-622-6001. If your insurance was declined because of an error on your part, (such as wrong policy number, etc), just email Pearce and Pearce with the correct information to resolve the problem. | ||
